Incorporated in 2001, as a non-profit organization, with a goal of providing benefits for its group members that could not be obtained on an individual basis.
The three primary benefits of membership with NAF-MD includes:
NAFMD NA245D Dental Plan
A top-rated HMO dental plan NA245D , offered by SafeGuard Health Plans, Inc., founded in Florida in 1985. SafeGuard Health Plans, Inc. is a Met Life affiliate. This plan is the best for someone who has a need for extensive work at one time. The monthly membership is higher per month but the overall savings will be much more because the Co-Pay is the lowest in the industry.
Vision Benefits
Administered by Coast To Coast Vision (CTC). Who has contracted with over 12,000 eye care locations nationwide to give you 20% to 60% discounts on eyeglasses, 10% to 20% discounts on contact lenses (excluding disposables), and other items offered at retail in the provider’s office.
CTC also contracts with Opthalmologists in selected markets across the United States to provide members with discounts on eyeglasses, contact lenses, eye exams and surgical procedures (including LASIK) where available.
Discount Drug Card
Administered by CareMark, this discount drug card allows members to enjoy excellent prices on prescription drugs. It provides a website showing the cost of each drug and local pharmacies in both your area and nationwide.
2. HumanaOne Dental and Vision Plans
C550 Plan Summary
A top-rated HMO dental plan C550, offered by Humana. This plan is the best for someone who has a need for minimal oral health care and normally only needs cleanings and checkups. The monthly membership is lower per month and it provides your preventive care at no cost to you. If you do need restorative procedures like fillings, you are covered with Co-Pay that will keep the costs down for you.
Humana PPO Plan Summary
A top-rated PPO dental plan, offered by Humana. This plan is the best for people who do not have a variety of network providers in their area or who want to pick their own dentist.
HumanaOne Vision Plan Summary
This is a stand alone vision plan that will save you money. Over 35000 participating locations that accept this plan.
